Introduction
Crisis Counseling Assistance and Training Program Immediate Services Program Application Materials
This application package contains the application materials for the Immediate Services Program (ISP) of the Crisis Counseling Assistance and Training Program (CCP), which is administered by the Federal Emergency Management Agency (FEMA) with consultation and technical assistance from the Substance Abuse and Mental Health Services Administration (SAMHSA) Center for Mental Health Services (CMHS) within the U.S. Department of Health and Human Services. The CCP provides supplemental funding to States for short-term crisis counseling services for eligible disaster victims in Presidentially declared disasters. The ISP may fund up to 60 days of crisis counseling services from the date of the disaster declaration. The application must be submitted to the FEMA Federal Coordinating Officer (FCO) for the disaster within 14 days of the Presidential disaster declaration.
Three entities are eligible to apply for and receive CCP funding after a Presidential disaster declaration: States, U.S. Territories, and federally recognized Tribes. It is recommended that the applicant review all the supplemental instructions before developing the application. Consultation and technical assistance for completing an ISP application is available from both FEMA and CMHS. What this Application Package Includes
The application package contains the key materials needed to complete an ISP application. These materials are described briefly below:

Standard Form 424 (SF-424) Request for Federal Assistance
The SF-424 is a Federal application form required by the U.S. Office of Management and Budget (OMB) for grant applications. The SF-424 is required for all applications for ISP grant assistance. Information is included in the supplemental instructions on how to complete this form. The SF-424 should be provided at the front of the ISP application and must be signed by the Governor's Authorized Representative (GAR) for the Presidentially-declared disaster.

Standard Form 424a (SF-424a) Budget Information: Non-Construction Programs
The SF-424a is another Federal application form required by OMB for grant applications. The SF-424a is required for all applications for ISP grant assistance. The SF–424a is a budget form that represents the total budget for a CCP.

ISP Budget Table Worksheet
The ISP Budget Table Worksheet is in Microsoft Excel format. It may be used to prepare the State and individual service provider budgets and narratives for the ISP application.
Applying for Longer-Term Crisis Counseling Grants
A separate application package has been developed for the Regular Services Program (RSP) of the CCP. The RSP can fund up to 9 months of services. A State applying for an RSP must submit a letter requesting an extension of the ISP until a funding decision has been made on the RSP application. The RSP application must be submitted to the FEMA Regional Director or Disaster Recovery Manager through the FEMA Individual Assistance Officer responsible for the disaster within 60 days of the disaster declaration. Because the application, review process, and funding procedures for the RSP are significantly different than those of the ISP, State applicants anticipating a need for longer-term crisis counseling services are encouraged to review the RSP application and supplemental instructions as soon as possible.
